If you talk about a Windows icon, yes, you need an image editor. Icons are computer graphics after all. This is no space to make "tv adds", but you can find one really good, shareware known as microangelo.

The avatars you see on yahoo or msn, are made with their own making tool, just look for a link within the website.

If you want to design your own avatar then i suggest using Adobe ImageReady (+Adobe Photoshop)
